TRUE Palliative Patient Story

A case manager with a local hospital was in the process of discharging a patient to the home. She believed this patient was near hospice appropriate and a good candidate for our TRUE Palliative Care program. One of our home health palliative nurses went and performed an evaluation on the patient. She determined he was not hospice appropriate at this time, but he had very specific home health skilled needs. Through collaboration with the original referral source, the patient, and his family we were able to offer him our TrueVine Home Health services. After several weeks, his outcomes are improving. He is gaining weight and getting stronger.

 

People aren't "one size fits all." That's the beautiful thing about palliative care. It's for anyone with a complex and serious illness who could use the extra layer of support.
