Getting started with your home health care
Your doctor may have referred you to home health services. Perhaps you have a surgery planned and know you will need assistance at home to recover. In any case, you are wondering about care at home. Please follow these easy guidelines:
If you've just been in the hospital: You can request that your doctor or case manager give you a referral for TrueVine Healthcare.
If you have not just been in the hospital: You can ask your doctor to contact TrueVine Healthcare or call us and we'll request the referral for you. When you call, our knowledgeable, caring staff will ask you the right questions to discover if home health may be the best option for your care.
Home Health care provides skilled nursing care, physical and occupational therapy, speech-language therapy service, and medical social services delivered to the comfort of your home. Home Health care is ordered by your doctor, and if you're leaving a facility (hospital or skilled nursing facility), the discharge planner may help transition your care to home. TrueVine nurses and other professional staff work with you and your doctor to evaluate your health care needs and write your plan of care, which tells you what home care services you will be receiving.
TrueVine staff will teach you and caregiver(s) (family or friends who are helping you) to continue any care you may need in the future, including wound care, therapy, and disease management. The goal is to help you reach and keep your best physical, mental, and social well-being.
Paying for home health
Medicare covers the cost of home health care. Private insurance, Long Term Care insurance and Veterans Benefits are also accepted. Some of the insurers we work with:

Does home health do it?
Do you need help managing your medications?
We teach you to safely take the right dosage at the right time and frequency. Our nurses help you learn more about what medicines will do for you and their possible side effects; nurses will also help you manage your medication schedule and communicate with your doctor regarding your status.
Do you need help understanding your disease or condition?
Our clinicians give you the important, up-to-date information you need to take charge of your health. We empower you to identify early warning signals and spot potential issues early.
Do you need your treatment progress monitored?
We help you follow your doctor's orders to speed your recovery time and make sure you keep appointments.

Perhaps not, but our team of healthcare professionals can evaluate your pantry and provide dietary education and nutrition management in an easy to follow manner.
Home health vs. general "home care"
TrueVine provides home health which is medically necessary care in the home. Home Health includes a broad range of medical care and support services for those who are recovering from a hospital stay, who are disabled or chronically ill and need medical, nursing, social, or therapeutic treatment. Home health care does not include companion sitters or nursing for extended periods of care time; the goal of home health is to provide education, medication management, wound care, short-term therapies, etc. to help people regain a more independent life-style and reach their health goal. Most home health care costs can be paid for by Medicare, Medicaid, long term care insurance, private insurance, or worker’s compensation.
Home Care (also known in our area as "private duty") provides companionship and homemaker services, in-home aides, nursing, or sitter services for extensive periods of time for patients who need additional help in the home--more than what their family and friends can provide. This help may be for long or short periods and is decided upon by the patient/family. Home care agencies charge for their services by the hour or accept private insurance or (in some states, but in most cases not in Oklahoma or Hawaii) Medicaid for services rendered. TrueVine Healthcare does nor provide this type of care but we can confidently refer you to many great agencies that provide this type of extended in home care for you or your loved one.
Your home health team
The TrueVine teams brings physician ordered medical care and therapy services to you, the homebound patient. We serve a 50 mile radius from our Norman office.
Skilled Nursing: works with you and your physician to develop your plan of care and provides education on disease processes and medication management along with many other medical services to promote wellness and independence.
Rehabilitative Therapists are trained in physical, speech or occupational therapy. They lead patients through physician ordered therapy and communicate with the doctor and other team members to adjust treatment as necessary.
Medical Social Workers help families locate and access community resources and benefits and address emotional needs that accompany illness and injury.
Certified Home Health Aides are available to assist with personal care. The can also guide you with your exercise programs and other activities of daily living. Aides may provide light house keeping assistance when personal care is also required.
Agency Trained Volunteers may be available to assist patients with companionship and light housekeeping, crafts, etc.
TrueVine Healthcare can arrange to have the pharmacy fill client’s prescriptions and will pick them up or have them delivered directly. We can monitor the refilling requirements and help obtain required prescriptions from client’s physician. Medicare and other insurers will cover the cost of certain medical supplies, like wound dressings, that are ordered as part of your care.
Durable medical equipment (DME)
Medicare and other insurers will usually cover assistive medical equipment that is necessary for your recovery. TrueVine Healthcare partners with local DME companies and can help arrange for home care medical equipment requested by your doctor such as:

Our social workers provide appropriate tools and methods for the patient to cope with personal issues or problems related to their illness or injury and provide a connection to many resources.
Therapists work with you at home to speed recovery from illness, injury or surgery. This reduces the risk of injury (or further injury) as opposed to transporting to and from a rehabilitation facility or clinic.
- Physical Therapists assist the patient in achieving a maximum level of functioning through a home exercise program
- Speech-Language Therapists administer a restorative program to help those patients with communication difficulties
- Occupation Therapists assist the patient in regaining daily skills needed to function independently at home or at work
